If you do not agree with any of the following terms, please do not use our website. ❖ APPOINTMENTS: A Heavenly TOC Mental Healthcare team member must schedule and confirm all appointments. The appointments may be scheduled via phone or email. Once an appointment is scheduled, the client will receive a confirmation email with the date and time of the appointment. ❖ FEES, Co-pay and Payment Fees for services provided by Heavenly TOC Mental Healthcare are due at the time of service. We take various insurances and accept cash, check, or credit/debit card payments. Payment plans may be available in certain circumstances but must be arranged in advance. Clients are responsible for all fees incurred for Heavenly TOC Mental Healthcare services. ❖ CANCELLATIONS, If you need to cancel or reschedule an appointment, please notify us at least 24 hours before your appointment. If you cancel your appointment less than 24 hours before your scheduled appointment time, there will be a $25 cancellation fee. Same-day cancellations and or no-shows will be charged a $50 fee. All fees will need to be paid before scheduling your next appointment. ❖ CONFIDENTIALITY, Heavenly TOC Mental Healthcare comply with all federal and state laws regarding the privacy and security of your health information.
